Golden Heart & Home was formed as a Group Home for elderly to provide a family home setting for adults who are unable to live alone due to physical, emotional or developmental impairments. Golden Heath & Home provides 24-hour care to the residents and is conveniently located in South Bend, Indiana. Residents receive meals, support, supervision, and assistance with personal care and living skills as needed.  Golden Heart & Home meets the need of adults who require periodic or regular assistance with activities of daily living but do not require nursing services.

With over 26 years in the medical field, bringing experience in Hospital setting, Long-term care, Skilled/Rehab, Home care, Hospice and Wound care. Starting as a CNA and furthering my education becoming a Family Nurse Practitioner. Specializing in Geriatric with the mission of providing person centered care.
